Myocardial adrenoceptors. Affected members of a kindred with POTS have a hypofunctional mutation of this transporter.8 To date, however, no study has tested whether decreased function of the cell membrane norepinephrine transporter characterizes the much more common sporadic form of this syndrome. Results of power spectral analysis of heart rate variability have been taken to suggest increased cardiac sympathetic tone or an excess of sympathetic over vagal tone sympathovagal imbalance ; .9 Such a physiological index cannot examine important determinants of cardiac sympathoneural function, including exocytotic release of norepinephrine from cardiac sympathetic terminals, norepinephrine synthesis, the density of sympathetic innervation, and in particular uptake-1 activity. Although evoked episodes in patients with NCS are associated with decreased sympathetic outflow to skeletal muscle and increased plasma epinephrine levels, which might help explain the characteristic skeletal muscle vasodilation, 10 studies of overall autonomic function between episodes have also so far not revealed consistent, specific abnormalities. During supine rest, plasma levels of norepinephrine, rates of peroneal sympathetic nerve traffic, and power spectra of heart rate variability are normal.1114 As in POTS, in NCS relatively little is known about tonic cardiac sympathetic innervation and function. In this study we examined cardiac sympathetic innervation and function in chronic orthostatic intolerance syndromes, addressing the following questions. First, does POTS or NCS involve altered cardiac release, neuronal uptake, or synthesis of norepinephrine or abnormal density of sympathetic innervation in the left ventricular myocardium? Second, if so, does heart rate relate to any of these abnormalities? Third, does pharmacologic stimulation of norepinephrine release from cardiac sympathetic terminals reveal altered presynaptic modulation of norepinephrine release or altered cardiac uptake-1 activity in these conditions? Fourth, does POTS differ from NCS in plasma levels of catecholamines during supine rest or orthostasis? To answer these questions, we measured the rates of entry of norepinephrine into coronary sinus plasma cardiac norepinephrine spillover ; 15 and cardiac extraction of 3H-norepinephrine and production of dihydroxyphenylglycol and dihydroxyphenylalanine16 at baseline, during exposure to mild lower-body negative pressure LBNP ; , and during intravenous infusion of the 2 adrenoceptor blocker yohimbine.17 The same patients had thoracic positron emission tomographic scanning after intravenous injection of the sympathoneural imaging agent 6-[18F]fluorodopamine to evaluate cardiac sympathetic innervation.18.

Fixed dorzolamide timolol, available in Europe since 1998, reduces IOP as much as 32%.20 It lowers IOP an additional 3.4 mm Hg compared with dorzolamide alone and an additional 2 mm Hg compared with timolol alone.20 Moreover, pressurelowering efficacy of fixed dorzolamide timolol is similar to concomitant administration of the individual components from separate bottles.21 When compared with a prostaglandin analog alone, fixed dorzolamide timolol and latanoprost monotherapy each provided 27% reduction in IOP from baseline, 22 corroborating an earlier study by Konstas.23 Fixed brimonidine timolol, which is discussed in greater detail later, is one of the newer fixed-combination products, having been introduced in 2005. The pressure-lowering effect of this agent is greater than the effects of either component alone.24 An interesting aspect of fixed brimonidine timolol is that the degree of conjunctival hyperemia observed is considerably less than that with brimonidine alone 14.4 % vs 22.8.

Veridical name identifications of eight food-grade liquid extracts of plant materials were learned to criterion 5 errors for 32 randomized presentations, with corrections ; during orthonasal presentations, then tested for identification with 16 retronasal and then 16 orthonasal presentations without corrections, using vapor phase presentations Pierce and Halpern, 1996, Chem. Senses, 21: 529 ; . Subjects had the veridical names of the eight odorants plus 12 other names. Retronasal sniffing was not used; orthonasal sniffing, not permitted. Subjects meeting the orthonasal criterion received the retronasal and next the orthonasal tests; then relearned the identifications to criterion with retronasal presentations and next received orthonasal and then retronasal tests. Results: 15 out of 17 subjects achieved the initial orthonasal criterion. Retronasal and orthonasal identification test errors did not differ significantly; overall error rates were 13%. Errors were rare 2 16 ; or absent for banana, peanut, and wintergreen. Lemon odorant identification errors were intermediate for both retronasal and orthonasal routes, but the rate of orange orthonasal testing errors was twice the retronasal identification rate. Thirteen of the 15 subjects met the retronasal learning criterion. Orthonasal and retronasal identification test errors did not differ significantly; overall error rates were 9%. Errors were rare or absent for banana, chocolate, cinnamon, coffee and wintergreen; those for lemon and especially orange, more common for orthonasal than retronasal testing. Conclusions: In the absence of sniffing, accurate odorant identification and substantial transfer of identification information can occur for orthonasal and retronasal routes. Some odorants may be more easily identified retronasally and tobi.

17 INTERRELATIONSHIP BETWEEN PERIODONTICS AND ORTHODONTICS P Diedrich, Department of Orthodontics, University of Aachen, Germany SHELDON FRIEL MEMORIAL LECTURE There is a wide range of indications for orthodontic tooth movement aimed at improving the prognosis of malpositioned teeth in the mixed dentition and of periodontally involved teeth. New regenerative periodontal procedures membrane technique, application of enamel matrix derivates ; have provided greater opportunities for gaining new attachment and have improved pre-orthodontic conditions for moving teeth into infrabony defects or for vertical movement of teeth with reduced bone support. The benefits and problems of a combined periodontal orthodontic treatment approach will be discussed with respect to preorthodontic mucosal grafting, guided tissue regeneration, loss of interdental gingiva, correction of crowding, reorientating of migrated and flared incisors, and gaining new abutment teeth by distalizing free-end premolars. 18 ORTHODONTIC ALGINATES AND DIMENSIONAL STABILITY: AN IN VITRO STUDY C Erbe, S Ruf, P Ferger, M Balkenhol, Department of Orthodontics, University of Giessen, Germany AIM: To assess the influence of three different storage conditions on the dimensional stability of three contemporary orthodontic alginates. MATERIALS AND METHOD: The following materials were tested: Jeltrate fastset, Dentsply Caulk mJT Orthotrace, Cavex mOT ; and Tetrachrom, Kaniedenta mTC ; . Eighteen specimens per material were fabricated using a stainless steel mould according to ADA Specification No. 18 ISO 1563 ; . Six specimens per material were stored in a humidifier HF ; , wrapped in a wet tissue inside a plastic bag PB ; and exposed to ambient temperature and relative humidity TH ; . The dimensional change was determined by measuring the distance of the vertical d-lines on the alginate impressions, using a travelling microscope M420, Leica, 25 magnification ; at baseline and after a storage time of 1, 2, 4, and 72 hours for HF and PB. The dimensional change for TH was measured at baseline and after processing for 15, 30, 45 and 60 minutes. The percentage of dimensional change was calculated from the values obtained. RESULTS: In the HF, dimensional change ranged from 0.18 to 8.28 per cent. When stored in the PB, dimensional change ranged from 2.8 to 1.95 per cent. All materials showed a high dimensional stability at baseline 0.4 per cent ; . When stored in the PB, mTC showed an expansion, while mOT and mJT contracted. After storage for 1 hour in the PB, dimensional change was significantly higher P 0.05 ; for mTC 1.77 per cent ; compared with mOT 0.09 per cent ; and mJT 0.05 per cent ; . After 72 hours storage in the HF, dimensional change was significantly lower P 0.05 ; for mOT 5.87 per cent ; compared with mJT 7.38 per cent ; and mTC 8.28 per cent ; . Surprisingly, dimensional change was less when the specimens were stored in the PB compared with the HF. CONCLUSIONS: Storage in a PB results in large dimensional stability for up to 4 hours for mOT and mJT. mTC was the alginate most affected by storage conditions. 19 OSSEOINTEGRATED VERSUS CONVENTIONAL ANCHORAGE TECHNIQUES: A RANDOMIZED CONTROLLED TRIAL I Feldmann1, L Zettergren-Wijk2, L Bondemark2, 1Orthodontic Clinic, Public Dental Health Service, Gvle, County Council of Gvleborg and 2Department of Orthodontics, Faculty of Odontology, Malm University, Sweden AIM: To evaluate and compare the capacity of four different anchorage techniques, the Orthosystem, Onplant system, headgear, and transpalatal bar, during the alignment phase after premolar extractions. SUBJECTS AND METHOD: After a sample size calculation, 83 patients aged 13-19 years ; were randomly allocated to receive reinforced anchorage with the Orthosystem, Onplant system, headgear, or a transpalatal bar. The inclusion criteria were patients in the permanent dentition, premolar extraction therapy in the maxilla, full fixed appliances, and anchorage reinforcement required on the maxillary first molars. The main outcome measures were time of alignment, cephalometric analysis of maxillary first molar movement, maxillary incisor movement, and sagittal growth changes of the maxilla. RESULTS: No significant difference in alignment time was found between the groups mean between 7.1 and 8.0 months ; . For the Orthosystem and Onplant groups the maxillary first molars were stable within the maxilla. In the headgear group a mean distal molar movement of 0.6 mm was found, whereas in the transpalatal group the first molars moved forward an average of 0.9 mm, i.e. anchorage loss. A significant difference P 0.001 ; in molar position was found between the transpalatal group and the other three groups. The maxillary incisors moved backward between 1.2 and 1.6 mm and the maxilla moved forward between 0.5 and 0.7 mm. No significant differences were found between the groups. e156 and tolcapone.
