One drug, Hycamtin, was originally approved for small-cell lung cancer and ovarian cancer, but can now be used conjunction with another drug to treat late-stage cervical cancer that is not likely to respond to traditional methods, such as surgery or radiation. FGM and Childbirth: Female genital mutilation FGM ; , the practice of removing part or all of the female external sex organs, and or stitching closed the vulva and opening of the vagina, has been linked to a marked increase in childbirth complications, including infant death, according to a study published in The Lancet. World Health Organization researchers examined more than 28, 000 birth records from Burkina Faso, Ghana, Kenya, Nigeria, Senegal, and Sudan. They found that women who had undergone FGM were 31 percent more like to have a c-section, 66 more likely to have infants that require resuscitation, and 55 percent more likely to experience infant death, compared to women who had not experienced genital cutting. Women who had undergone FGM were also more likely to bleed heavily during delivery and to require surgery and or longer hospital stays. Teen Sex in the U.S.: A recent study published in the Archives of Pediatrics and Adolescent Medicine, found that 41 percent of girls interviewed said they had experienced unwanted sex. Another 10 percent said a partner forced them to have sex, and five percent said they had sex after being offered money or gifts. The study included 279 girls age 14 to 17 from Indianapolis, IN. According to researchers, girls who engaged in unwanted sex were less likely to use condoms and had a higher rate of pregnancy. Another rare presentation of Wilson's disease common in India is `osseomuscular' with bony deformities suggestive of renal rickets8. Osteomalacia, spontaneous fractures and arthropathy are also seen.8, 9, 10 In the patient, the presence of polyuria, glycosuria and aminoaciduria point to a proximal renal tubular dysfunction. The sudden development of renal shut down within 24 hr despite intensive care is noteworthy. The effect of dehydration and persistent prolonged hypokalemia on dysfunctional renal tubules could have precipitated the acute renal failure in this child. Severe and persistent hypokalemia per se could lead to a variety of renal disturbances like decreased renal blood flow and Glomerular Filtration rate GFR ; , defect in renal concentrating capacity etc. 11 There are reports of rhabdomyolysis due to hypokalemia resulting in acute tubular necrosis12. Renal disturbances in Wilson's disease, even though well-documented, is rarely the presenting feature and almost never the cause of mortality. This patient had renal disturbances and hypokalemia of unusual severity even before any definitive therapy for Wilson's disease could be started. In patients presenting with flaccidity and hypokalemia, renal tubular dysfunction due to Wilson's disease should also be considered in the differential diagnosis. REFERENCES.

Penicillamine pharmacokinetics
The administration of penicillamine to rats causes a disturbance of collagen metabolism that resembles t, hat seen in lathyrism 6, 36 ; . The changes induced seem to be quite specific for soft tissue collagen, and the severe osseous abnormalities seen in lathyrism are not observed after penicillamine. The collagen changes caused by penicillamiue are reflected by pronounced weakening of t, he mechanical strength of soft tissues. Rats maintained on penicillamine for 3 months showed such tissue friability that in many instances, when restrained by their tails, they shed the entire skiu sheath covering this structure. Tracer amounts of 14C-prolinc, when given to rats receiving penicillamine, are incorporated int, o the more recently synthesized collagen that extracted by 0.15 M NaCl ; at a normal rate. The specific activity in the treated animals is lower because of the isotope dilution effect of the accumulating collagen, but upon correction for the pool size the differences between groups are not significant. The turnover time for this fraction in normal rats is approximately 20 hours, and for the 0.5 M NaCl fract, ion it is 42 hours. These parameters cannot be evaluated adequately in the shapes of the corresponding the treated rats. Although curves indicate an initial drop in radioact, ivity, the label begins to accumulate without any indication of turnover. The ability of the label to appear in the hydroxyproline of the insoluble collagen is very markedly impaired by penicillamine. Bone marrow progenitor cells in patients with RA. Magnetic resonance imaging has also demonstrated the presence of significant bone marrow edema shortly after the onset of RA 88 ; Histopathologic reports as well as MRI have demonstrated significant subchondral bone marrow inflammation in patients with another common arthropathy, ankylosing spondylitis, which primarily affects the axial skeleton 89 ; . There have been six doubled-blinded placebo-controlled trials of bisphosphonates in rheumatoid arthritis, four with pamidronate, one with alendronate, and one with clodronate. In the first study, forty patients were randomized to either 30 mg of pamidronate by monthly intravenous infusion or placebo 90 ; . There was no significant effect on disease activity as judged by clinical or laboratory criteria, although there was a significant decrease in the articular index in both groups. There was also no effect on radiologic progression of disease. However, all patients had already been stabilized on a disease-modifying agent penicillamine and this was not discontinued prior to study entry. In contrast, a second study examined the effects of a single intravenous infusion of placebo, 20 or 40 mg of pamidronate, in thirty patients with active RA 91 ; . twenty-one days after the infusion, there was significant clinical improvement with both doses of pamidronate and improvement in the ESR and C-reactive protein after the 40 mg dose. This same group then examined 105 RA patients randomized to receive either 300 mg oral pamidronate per day or placebo for three years 92 ; . Disease activity improved significantly in both treatment groups and there was a gradual increase in mean radiologic scores in both treatment arms. The interpretation of this data is complicated by the fact that: 1. The study was not primarily set up to evaluate efficacy but changes in bone mineral density, 2. Half the patients were allowed to take disease modifying anti-rheumatic drugs, and, 3. There was a high withdrawal rate in the pamidronate group because of side effects. Despite this, a change in concomitant anti-rheumatic drug therapy was considered necessary because of failure to control disease activity more commonly in placebo patients than in those receiving pamidronate. Finally, Maccagno et al 93 ; examined oral pamidronate 1, 000 mg per day in 27 patients randomized to receive either drug or placebo for one year. Despite more severe disease at baseline, pamidronate treated patients experienced significant improvement in some clinical a.m. stiffness ; and laboratory ESR ; parameters of disease. In addition, progression of periarticular erosion was suppressed by pamidronate as compared to the deterioration observed in the control group. A small double-blinded study of thirty-six patients with RA, randomized to either placebo or 1600 mg of clodronate, demonstrated a significant fall in the CRP by one month and a trend towards improvement in the articular index at twenty-four weeks in patients who received clodronate 94 ; . In summary, different patient selection criteria and clinical trial designs, differences in therapeutic regimes, small patient numbers and consequent lack of statistical power, do not allow any firm conclusions to be drawn although there appears to be marginal evidence for efficacy when higher doses of pamidronate have been examined.
