Wonder no one there wanted to even hear about any connection between Lupron and problems'--but how many other patients know of conflicts of interest in their circumstance s ; and who will tell them? 4. A Brief Overview of the ``Hazardous'', ``Commonly Prescribed'' Agent Lupron Lupron, referred to as a GnRHa gonadotropin releasing hormone analog agonist [and also previously referred to as LHRH] ; , will be the focus of my comments as it is one of the most commonly used agents, but it should be recognized that numerous other GnRHa's as well as the newer GnRH antagonists are being used in superovulation of women--and the risks from all of these agents should be taken into consideration. Thousands of women have become seriously ill after taking Lupron and there have also been complaints about other GnRHa's, such as Buserelin, Synarel and Zoladex and the alleged safety and mechanism of action of these drugs needs attention. A National Lupron Victims Network NLVN ; was founded in 1993, and when I learned of their existence in 1994, they were the only entity who was interested in the results of my searches into the medical literature; and this information, along with many other sources of information, continues to serve as the `clearinghouse' of information on the risks of Lupron at their website, lupronvictims . The NLVN began a visit counter on January 1, 2000: as of 3 there were 2, 119, 422 hits made to this site. While the NLVN provides detailed information on Lupron, other internet sites contain public message boards about problems after Lupron, i.e., Delphi message boards such as `Julie's After Lupron Page' Julie's Page ; , and AOL message boards, among many others. The initial patent filed for Lupron involved ovulation induction Patent #4, 005, 063 ; , and Lupron has been used in drug company funded studies to induce ovulation i.e., Segal, 1992 ; . Lupron has become the ``standard of care'' for some 15 years, for a variety of reasons, including to maximize number of eggs produced. A multitude of many, including numerous Internet pharmacy websites, hawk Lupron as a ``fertility medication'' . yet the FDA has never approved Lupron for infertility, or fertility treatment, or IVF treatment, or any variant of IVF or ART. According to the Physician's Desk Reference, the FDA classifies Lupron as a Pregnancy Category X drug, meaning any woman who is or who may become pregnant should not use. Lupron is a known teratogen Shephard, 1992 ; , and Lupron is a known developmental and reproductive toxicant Scorecard ; . NIH and OSHA place Lupron leuprolide ; on its list of hazardous drugs NIH, OSHA ; . Yet, inexplicably, medical literature reports Lupron to be the most commonly prescribed and ``prevalent choice'' of GnRHa used in fertility treatment Keenan, 1991; Martin, 1994 ; . Medical literature regularly refers to Lupron as an antineoplastic and chemotherapy, with some references characterizing Lupron as an antineoplastic hormone. Yet, according to deHaen modified American Hospital Formulary System, Lupron is not listed in the antineoplastic hormone category Classification No. 10: as are the drugs Tamoxifen, Megestrol, Flutamide ; --but rather Lupron is listed in the antineoplastic OTHER category Classification No. 10: 00.12, listed along with Interferon ; deHaen, 1995 ; . No one seems to know what the ``other'' in Lupron is! But it is known that Lupron was originally approved out of the FDA's Office of Biologics Research and Review. NDA [New Drug Application] 19010 ; . Clinical studies conducted by the manufacturer to evaluate Lupron's efficacy and `not' safety ; in fertility treatment and in IVF occurred between 1988 through 1992, according to Abbott Annual Reports see also FDC, 1988 ; . The ``IVF clinical trials'' and ``fertility treatment clinical trials'' using Lupron ``were discontinued'', according to the manufacturer Abbott correspondence, 1995 ; , and I've been unable to learn whether these Lupron IVF and Lupron fertility trials were discontinued because of efficacy reasons, safety reasons, both reasons, or other reasons. Lupron has never gained FDA approval for any type of fertility treatment, however, Lupron did gain FDA approval for use in women for pain management of endometriosis in 1990--yet the clinical studies for these approvals are a joke--conducted on a handful of women, by paid investigators, and with an endpoint of establishing Lupron's efficacy in pain management of endometriosis while the women in these studies were simultaneously allowed to take narcotics, including Dilaudid and parenteral narcotics. These women were also expected to `recall and record their adverse events at the end of the study month', yet were not informed that Lupron was known to affect memory NDA 19010; see also Newton, 1996 ; . Problems with this trial alone could fill this document--never mind attempting to address the numerous and gross problems evident within other Lupron NDA's. Lupron is alleged to cause menopausal symptoms such as hot flashes and headaches, and Lupron's categorization as a ``hormone'' is an allusion that is frequently conveyed to women. Women are often told by the physician, and TAP continues to!


Subjects were 14 healthy volunteers 10 males, 4 females, mean sem age 37.3 3.4 yr ; . All subjects underwent complete medical history, physical examination and screening laboratory examinations, a psychiatric interview with a board-certified psychiatrist D.M. ; , as well as a structured clinical interview SCID ; for DSM-IIIR to exclude current or past psychiatric illness. No subjects were on chronic medication. The study was approved by the National Institute of Mental Health Institutional Review Board, and all subjects gave informed consent to participate. HPA axis function was assessed in each subject details below ; , following which imipramine administration was begun. After 6 weeks of daily imipramine, with doses adjusted to maintain serum imipramine.

Long-term treatment with GnRH-A supplants the effect of physiologically pulsatile endogenous GnRH and is thought to down-regulate its receptors in the pituitary gland, leading to castration levels of testosterone within 3 weeks.15 It is well recognized that GnRH-As initially cause a surge in testosterone and can cause a "flare" reaction in patients with metastatic prostate cancer. This is due to an acute stimulation of prostate cancer growth by elevated levels of testosterone. A placebocontrolled trial has shown that androgen receptor antagonists lower the amount of bone pain with initiation of GnRH-A therapy for patients with meta.

Docetaxel has been associated with increased lacrimation that is thought to result from canalicular stenosis. Docetaxel concentrations were measured in tears in six patients; lacrimal concentration ranged from 14 to 70% of plasma concentration. This side effect may be more common with weekly versus threeweekly docetaxel. Artificial tears and or ophthalmic steroids have been used with variable success. Although increased lacrimation has reportedly improved following discontinuation of docetaxel, dacryocystorhinostomy with placement of permanent tubes has been required in some patients. Potential Drug Interactions: The metabolism of docetaxel may be modified by the concomitant administration of compounds that induce e.g., phenobarbital ; or inhibit e.g., itraconazole, ketoconazole, erythromycin ; cytochrome P-450 3A4. Docetaxel has also been shown to inhibit CYP 3A4 in studies with human liver microsomes. Thus, docetaxel might increase the concentration of other drugs metabolized by this isoenzyme. To date, there is little documentation of such interactions in patients. 12.5 LHRH agonists At the physician's discretion, equivalent LHRH agonists other than those listed below may be used. 12.5.1 Leuprolide Acetate Please refer to the FDA-approved package insert for leuprolide for product information, extensive preparation instructions, and a comprehensive list of adverse events. Availability Leuprolide acetate is commercially available as a kit or prefilled dual-chamber syringe containing 7.5 mg or 22.5 mg and intended for administration every four weeks or every 12 weeks. Lupron Depot ; . The leuprolide in these single dose preparations is present as lyophilized microspheres. Storage and Stability Intact kits and syringes should be stored at room temperature. Once reconstituted using the diluent provided in the kit or syringe, leuprolide suspension is stable for 24 hours. Preparation Reconstitute the microspheres with the diluent provided in the kit or release the diluent in the syringe into the microspheres. Gently shake the reconstituted product to yield a uniform suspension. Administration Leuprolide 7.5 mg or 22.5 mg is administered via intramuscular injection every 4 weeks or every 12 weeks, respectively. Toxicities Common toxicities are mostly related to the effects of decreased serum testosterone. Hot flashes are seen in more than 50% of patients. Gynecomastia, impotence, and decreased libido are also related to decreased testosterone, although they do not occur as often as hot flashes. Equilibrating mechanisms for sodium exists in hypertensive rate1. This imbalance may be masked by the presence of a new equilibrium so that some form of loading is required to reveal its presence. In certain forms of experimental hypertension in the rat, notably those produced by DCA or Compound F, the disturbance is so marked that the imposition of a sodium load may lead to an increase in the total sodium output as well as an acceleration in the rate of excretion. In other forms, notably those produced by renal compression, by 9a-chlorohydrocortisone and as a spontaneous phenomenon in age, the disturbance seems to be less intense so that the imposition of the load maj' not be followed by any manifest increase in the total sodium output, although there is an increased rate of excretion. The well defined acceleration in the rate of sodium output is thus a disturbance in pattern common to all forms of hypertension so far studied. In our previous study we raised the question of whether the metabolic disturbance is prerenal, so that the kidney is involved only as a compensatory regulator, or primarily in the kidney itself. The fact that the abnormality may be completely reversed by Pitressin, an extract with definite renal effects, suggests and levorphanol.
